In completing this form, you are indicating that you would like a formal Department of Internal Medicine Chair's Letter sent on your behalf to the ERAS application system. This letter will include a summary of performance during your medicine clerkship as well as a brief paragraph of support. The letter will be signed by Drs. Barrett (associate chair of education), Ashley (clerkship director), Jessee (acting internship director), and Sime (chair).

This form will assist us in developing the letter and will provide the background for a brief meeting between you and either Dr. Barrett, Dr. Ashley or Dr. Jessee. These meetings will be scheduled between May and September of this academic year. This form will also assist us in advising you during the residency application process.
