The purpose of this survey is to help with the evaluation of the effectiveness of the Evidence-Based Practice Portfolio along with the education that is provided along with the use of this portfolio through self-assessment. This survey will gauge your feeling towards EBP, your knowledge of EBP and your use of EBP in the clinical setting. This survey will be given at three separate points in the course of carrying out your EBP project while utilizing the online portfolio.
