Screening Survey for Teens and Parents Study-2 (TAP-2)

You are invited to complete this pre-screening survey is to see if you might be a good fit for the Teens and Parents Study -2 (TAP-2) research study. The purpose of this 5-minute screening survey is to see if you meet the criteria to be in a research study. If you agree to take the survey, you will be asked questions about you, your teenager and his/her autism spectrum disorder diagnosis, other medical conditions your teen may have, and your teen's cognitive functioning. You have the option to complete this survey as an interview over the phone. If you would like to complete this survey over the phone, please call the CHAMP lab at 804-828-2053. Completing this screening survey does NOT obligate you to participate in the TAP research study. If your answers show that you qualify, a member of the TAP-2 research team will contact you.
