As a family who is receiving or has recently received Early Intervention services, you are invited to participate in a research study looking at how Early Intervention providers make decisions about service delivery. In this survey, we use EI to mean Part C Early Intervention. By completing this survey you are helping make Early Intervention-Part C more inclusive and effective for the next generation of families. 

 

In this research study, you will be asked to complete an online survey. The survey is divided into 4 sections: 1) questions about your family demographics, 2) questions on your opinion of the family centeredness of EI, 3) questions on the activities performed during EI, and 4) questions on how social and environmental factors may influence EI.  

Your participation in this study is voluntary and your identity will not be disclosed. Your EI services will not be affected by your choice to participate or not participate in this study. Your providers will not know if you participated in the study or your responses. Data gathered from this study will be combined with other participant's responses and analyzed as a group. Results from this study will be shared in aggregate form and all identifying information will be removed.

 

This survey should take approximately 15-20 minutes to complete. You can complete the survey on a computer or phone. If using a phone - we recommend putting the phone in landscape (on its side), this will let you easily see all answer choices. You can skip any question or exit the survey at any time. If you do not complete the survey in one sitting, please save the code and link provided when you exit in order to return to the survey - do not begin a new survey.

 

Please answer each question as honestly as possible. Don't feel like you have to spend a long time over each question. The first answer that comes to  you is usually the best one. If you aren't sure how to answer a question, choose the best answer from the options given. Some of the questions may be sensitive. You can choose not to answer. 

 

If you complete at least 85% of the survey, you are eligible to receive a $20 e-gift card as a thank you for you time and input. In order to receive the gift card, you will need to share your email address at the end of the survey. If you do not want to link your email to your survey responses, you can choose not to provide your email address - but then we will not be able to send you the gift card. At the end of the survey we also ask if you would like to participate in a follow-up interview to share more information.

** For caregivers with multiple children in EI - try to answer this survey with one child is mind. If you want to fill out the survey for your other child (or children) - at the bottom of the survey select "Yes" when asked "Do you have other children in EI that you would like to fill out the survey for?" This will generate another set of questions for you without making you answer your family information again.
